California State University (CSU) requires all students to be current on a number of immunizations and to complete a risk assessment for tuberculosis (CSU Executive Order 803).
This executive order and its requirements apply to undergraduate and graduate students, students matriculated in self-support degree programs and K-12 students participating in concurrent enrollment courses entering the California State University.
Required Immunizations and Screenings
- Hepatitis B (Hep B) - three (3) doses for students age 18 years and younger.
- Measles, Mumps, and Rubella (MMR) - two doses (2) with first dose on or after first birthday; OR positive titer (laboratory evidence of immunity to disease).
- Meningococcal Disease (Serogroups A, C, Y, W-135) - one dose (1) on or after age 16 for all students age 21 or younger.
- Tetanus-Diphtheria-Pertussis (Tdap) - one (1) dose after age 7.
- Varicella (Chickenpox) - two (2) doses with first dose on or after first birthday; OR positive titer. History of contracting the disease does not meet compliance.
- Tuberculosis (TB) Screening/Risk Assessment.

Upload one of the following documents to myHealth with your name and student ID number clearly indicated on each document submitted:
- Photocopy of your childhood immunization record
- Photocopy of your California High School transcript if immunization information is documented on transcript
- A copy of a lab report showing proof of immunity by blood titer
If you believe you have a medical reason for not getting a required shot (MMR or Hep B), please contact your primary care provider to obtain documentation of your medical exemption. For other questions related to immunizations, please contact Student Health Services at 415-338-1251.

As of December 22, 2021, the Chancellor's Office announced it will require faculty, staff, and students to receive booster shots to be fully immunized against COVID-19 and comply with the CSU Vaccination Policy. For those in California, visit MyTurn.ca.gov to find a clinic near you to schedule your booster. If you are outside of California, please visit Vaccines.gov.
Noncompliant students also will have a hold placed on their account that will prevent registration for the following semester.
