Vaccinations and Immunizations

In accordance with the California State University Policy Executive Order (EO) 803[1] on Immunization requirements, CSU policy on COVID-19 Vaccinations and Other Safety Measures[2], and the Center for Disease Control and Prevention (CDC) Immunization Schedules[3], San Francisco State University (SFSU) has implemented the following Policy on Immunization requirements and recommendations.

 

 

 

Categories of Requirements:

  • All Students (ages 18 and younger)
  • All New Incoming Students
  • Students in SFSU Campus Housing
  • Exemptions

 

All Students

All students ages 18 and younger are required to provide proof of full immunization against hepatitis B (HepB) prior to enrollment per California Health & Safety Code (HSC), Sections 120390-12039.7.[4]

All New Incoming Students

TB Risk Screening TB All NEW incoming students are required to complete a TB Risk Screening[13][14] prior to arrival on campus and in conjunction with verification of pre-matriculation immunization requirements.Only those students with identifiable risk factors for exposure to TB and/or for TB disease will be tested.

 

Students in SFSU Campus Housing

SFSU, in accordance with California Department of Public Health (CDPH)[6] recommendations and the CDC, requires that students seeking eligibility for SFSU campus residential housing are required to complete the requirements listed for all students as indicated above (i.e. HepB), and provide proof of full immunization for the immunizations below:

** Antibody level or titer blood test may be used to validate immunity to MMR, VAR, and HepB. Laboratory test costs

 

Exemptions

A student may be exempted from any requirements in this policy as described below:

Medical Exemption: due to a medical condition for which an Approved Vaccine presents a significant risk of a serious adverse reaction. Any medical Exemption must be verified by a certified or licensed healthcare professional.

Religious Exemption: due to either (i) a person's sincerely held religious belief, observance, or practice, which includes any traditionally recognized religion, or (ii) beliefs, observances, or practices which an individual sincerely holds and that occupy a place of importance in that individual's life, comparable to that of traditionally recognized religions.

RECOMMENDATIONS

The CSU recommends that students are current for the immunizations listed below. Immunizations prevents death worldwide and students are strongly encouraged to prevent diseases, protect health throughout the lifespan, and help to prevent and mitigate outbreaks.

MEETING THE REQUIREMENTS

Students will be required to submit their immunizations and documentation by the first day of classes for the fall semester and by the first day of classes for the spring semester.

Students may fulfill immunization requirements by submitting medical documentation as proof of immunization online through the secure MyHealth patient portal.

 

EXEMPTIONS

Students are permitted to seek exemptions to the above immunizations for medical or for sincerely held religious beliefs. Students seeking an exemption will be required to submit an Exemption Request Form to the university. Exemption requests must be submitted by the same deadlines as outlined above.

 

ADDITIONAL DISCRETIONARY REQUIREMENTS

In the event of a local or campus - based outbreak of a vaccine-preventable disease, the local public health department and/or the CDPH is authorized to require immunizations beyond those currently required. SFSU will consult with the appropriate public health authority in case of an outbreak.

 

ACADEMIC PROGRAM AND FIELDWORK OR CLINICAL PLACEMENT REQUIREMENTS

All students enrolled in health care fields of study or teach education programs are required to provide documentation regarding the completion of immunizations and health screenings as established by the state licensing agencies or clinical placement sites or fieldwork sites. Students should contact the academic program for the specific required immunizations, health screenings ( e.g., TB screening and physical examination) and any other fieldwork or clinical placement requirements.

 

HOLDS

Students who are not compliant with the university's requirements immunization and tuberculosis (TB) risk screening requirement may have a registration hold placed on their record. The hold may be implemented for the second semester of their enrollment at the university and will prevent registration in classes offered in future terms.

 

OPERATIONAL RESPONSIBILITY

Student Health Services (SHS) will administer the immunization requirement program on behalf of the university. SHS will also be responsible for advising students on how to obtain required immunizations through private health care providers, local health departments, or SHS. Questions regarding the requirements should be directed to SHS.

 

EFFECTIVE DATE

This SFSU policy on Immunization Requirements & Recommendations is in effect for all SFSU students with the start of the Fall 2023 term

To upload Immunization records

  • Log into myHealth
  • Select Forms & Immunization Requirements
  • Upload a picture of your immunization records under Immunization Documents.
  • After uploading or submitting your records, click in the name of each immunization” (Be sure to include the date of each vaccination dose/titers.
  • Click done.

To upload COVID-19 Vaccination / Booster Card:

  • Log in to myHealth (button above); follow prompts
  • Click "Enter my COVID-19 vaccination information"
  • Upload a scanned or photo copy of your Vaccination/Booster card.  If you are uploading a new document with your Booster information, your previous entry will not be deleted. Please ensure your name and date of birth are on each upload.
  • Your card must be no larger than 2MB, in English, and readable.
  • Enter the dates and types of vaccines you received.  *If your booster was Pfizer, select the Pfizer option in the list (does not state "Booster").
  • Click "Done" (status will be "non-compliant" until verified).
  • Check back in "Forms & Immunization Requirements" in 7-10 days to see if your uploads have been verified and your status updated to "Compliant."
