International Student Health Insurance

All international students and scholars must have health coverage during their stay in the United States (United States Information Agency Rule 22 CFR Part 514 and California State University Executive Order Number 622).


Students who do not show proof of insurance will receive a Registration Hold, and will not be able to register for classes.



Insurance Requirements

SF State 2014-2015 Insurance Policy Minimum Requirements

Insurance Broker must certify that all of the following conditions are met:

  • Policy must be in English.
  • All coverage limits must be in U.S. dollars.
  • Medical benefits of at least $250,000 for each accident or illness.
  • Full cost of medical evaluation.
  • Full cost of repatriation of remains.
  • Coverage for the entire academic year.
  • Must cover pre-existing conditions, with a wait person of no more than six months if attending the full academic year.
  • Minimum 75% coverage for each accident or illness.
  • No capped benefits, e.g. $1,200 per day for hospital room.
  • Maximum deductible of $100 per condition per plan year.
  • Maximum out-of-pocket expenses $2,500 or less.


California State University (CSU)-Sponsored Policy

All of these requirements are met by the California State University (CSU)-sponsored policy with Aetna, through Wells Fargo Student Insurance Division.


Wells Fargo Student Insurance Division will automatically notify the SF State Registrar to release the insurance hold upon purchase.


Other Health Insurance Policies

If you do not purchase the CSU-sponsored policy, your insurance policy must meet the SF State 2014-2015 Insurance Policy Minimum Requirements.


Make sure your insurance company or broker gives you written confirmation or an affidavit on their official letterhead that their policy meets all of the 2014-2015 requirements before you purchase the plan. Send a copy of this document to the Registrar.


If the insurance broker does not provide you with written confirmation or an affidavit, you may be purchasing a plan that is noncompliant. You will receive a Registration Hold.



Affidavit - A written declaration made under oath before an authorized notary or officeholder.

Letterhead - a sheet of stationary with name and address of the organization printed at the top.

How to Purchase CSU-Sponsored Health Insurance Policy

Credit Card Purchase

  1. Go to the CSU Healthlink SF State Plan.
  2. Go to 2014-2015 San Francisco state University-International (1st section).
  3. Click on "Enroll" and select "Annual".


Money Order or Cashier's Check Purchase

After you complete the enrollment online:

  1. Download the enrollment card for the appropriate term.
  2. Make the money order or cashier's check (in U.S. funds drawn on a U.S. bank) payable to Wells Fargo Student Insurance Division. No personal checks are accepted.
  3. Mail payment and enrollment form to:

Assist America Emergency Travel Insurance

If you are a citizen of Sweden or Norway please submit the following in advance of arrival:

  • A copy of your insurance card with your name on it.
  • Your SF State student identification number.
  • Your email address or phone number

Coverage for Optional Practical Training

International students interested in purchasing insurance coverage for an Optional Practical Training period must give Wells Fargo Insurance Services a copy of your "Employment Authorization Document" (EAD) card by:
