Q.  Do you need me to send my medical records to you?

A.  If you have health concerns that require ongoing monitoring, please have your current healthcare provider send you copies of your medical records. Keep your copy and send / deliver another copy to the SHS. 

When you arrive on campus, please schedule an initial visit with one of our clinicians by calling  415-338-1251.

Q.  Does SFSU require a physical before starting school? 

A.  Physical exams are required only for students participating in intercollegiate athletics.  The SFSU Athletics Office staff will schedule a pre-participation physical at SHS for each team athlete.

Q.  Which immunizations do I need before coming to college?

A.  Measles, Mumps, Rubella (MMR) and Hepatitis B are required for all SFSU students.

Q.  Where do I submit required immunization records?

A.  The SFSU required immunization records should be mailed, faxed, or a copy brought in to the SFSU Registrar’s Office, Student Services Building, Room 101. 

If needed, you can get an immunization at the SHS during Immunization Clinics. Some vaccines such as Meningococcal Vaccine may need to be special ordered.

Q.  How do I pay my bill and how much will it cost?

A.  The Student Health Fee (included in your tuition) covers most medical care provided at SHS except for items such as medications, lab tests, and immunizations and vaccines.  Please see the SHS Charges webpage.  Charges incurred at the SHS are billed to your student’s Bursar account.  Itemized bills to submit to your student’s health insurance for reimbursement may be printed out at the SHS secure site, myHealth.

Q.  How do I get health insurance through SFSU?

A.  A complete health insurance plan has been selected by SFSU and is available for students to purchase. Policy details and enrollment deadlines are available at csuhealthlink.com.

Q.  Where is SHS located?

A.  The Student Health Service is located below ground adjacent to the Psychology and Ethnic Studies Building, directly across from Burk Hall in the northwest corner of the SFSU campus.

Q.  How do I make an appointment?

A.  You may schedule an appointment in person, online (select types of appointments) or over the phone by calling (415) 338-1719. We encourage you to schedule your appointment over the phone to expedite your services. For your convenience 80% of our appointments are same-day appointments.

Q.  I just had surgery. Can I get help getting to and from class?

A.  Sometimes students experience temporary impairments that limit their mobility to travel around campus. The Disability Programs and Resource Center (DPRC) website includes information about transport assistance and other assistance for students with permanent or temporary disabilities.

Q.  Do I need to fill out any forms before I make an appointment?

A.  Yes SHS asks that you complete a health history form before coming in for your appointment. You can find it by clicking on myHealth and signing in to your secure medical information page. You will find the Health History Form in the FORMS section.
