Immunizations Required
Prospective SFSU students should send their completed "proof of immunization form" of the required immunizations to the Registrar's office before registration. Send your immunization form to:

Failure to comply with the immunization requirements will result in a registration hold. The Student Health Services does not generate registration holds and does not remove registration holds. For Registration Hold information please contact the Registrar's Office at 415-338-2350.

Required Immunizations
The San Francisco State University (SFSU) immunization policy requires that all students except those who were enrolled in a California public school for the seventh grade or higher on or after July 1, 1999 shall provide proof of immunization against Measles, Mumps and Rubella before being allowed to register for classes. In addition SFSU students who are 18 year old or younger will need to show proof of Hepatitis B immunization before registering for the first semester of classes.

Based on medical documentation the SHS provides medical waivers - please see the Immunization Waiver Form. SF State's Registrar's Office provides for waivers based on medical considerations and religious or personal beliefs.
