Prospective SFSU students should send their completed "proof of immunization form" of the required immunizations to the Registrar's office before registration. Send your immunization form to:

Failure to comply with the immunization requirements will result in a registration hold. The Student Health Services does not generate registration holds and does not remove registration holds. For Registration Hold information please contact the Registrar's Office at 415-338-2350.
The San Francisco State University (SFSU) immunization policy requires that all students except those who were enrolled in a California public school for the seventh grade or higher on or after July 1, 1999 shall provide proof of immunization against Measles, Mumps and Rubella before being allowed to register for classes. In addition SFSU students who are 18 year old or younger will need to show proof of Hepatitis B immunization before registering for the first semester of classes.
Medical Waivers: If you have a medical reason for not getting a required shot or immunization (MMR or HepB), SHS will fill out your waiver form.
Turn in your completed Medical Waiver to the SF State Registrar's Office.
Religious or Personal Beliefs Waivers: If you have a religious or personal reason for not getting a required shot or immunization (MMR or HepB), go to the Registrar's Office.
